Medical Plan Options
Hardin Memorial Health offers two medical plan options along with the option to waive participation if you have other coverage. Prescription drug benefits are included in each plan. Regular full-time and regular part-time employees (authorized at least 40 hours per pay period) may elect health insurance coverage. You can choose coverage for yourself, you and children, you and spouse or for the entire family. Premiums are based on the plan you select and the number of dependents you cover.
Prescription Drug Coverage
To use the drug card, just go to a participating pharmacy and present your insurance card. You will pay the required co-payment or coinsurance percentage for your prescription. With the mail order benefit, you may obtain up to a 90-day supply of maintenance medications for the price of two months’ supply. Some specialty drugs are not covered at the retail pharmacy level and must be ordered by mail and are subject to a separate co-payment. These are usually high cost treatments for conditions such as multiple sclerosis, arthritis, and cancer. Insulin, Byetta and Imitrex, however are injectables that you can get at the retail pharmacy.
Hardin Memorial Health offers two dental plan options. Both plans provide comprehensive coverage and encourage preventative care. You can choose coverage for yourself, you and children, you and spouse or for the entire family. Premiums are based on the plan you select and the number of dependents you cover.
Hardin Memorial Health offers employees vision benefits with Baltas Vision, LLC. Benefits provided are an eye exam and a complete pair of prescription eyeglasses, to include standard plastic lenses in single vision, lined bifocals or lined trifocals at a low co-payment rate, if using a network provider. Coverage is available at non-network providers but at a greater co-payment. Plan benefits are payable once every 12 months. Contact lenses are available in place of the frame and lens benefit. Health Risk Assessment
HRAs, Disease Management and On-site Acute Care
Hardin Memorial Health provides an annual health risk appraisal to all employees. The Employee Health office coordinates this voluntary screening. Employee Health has an on-site nurse practitioner who provides acute care in the event of employee illness or injury during daytime work hours.
Flexible Spending Accounts
Flexible Spending Accounts allow employees to pay for certain health care and dependent care expenses on a pre-tax basis through payroll deductions. Hardin Memorial Health offers a health care FSA and a dependent care FSA. The plan features a convenient debit card for immediate reimbursement.
