Pre-registering helps to expedite your visit by eliminating the process at the time of your appointment.
Allows us to communicate with your insurance company to verify benefits and eligibility, start pre-certification, obtain referrals and pre-authorization if needed, and resolve any insurance concerns before your admission date
We can inform you of any financial obligations beforehand so you can plan for the unexpected.
When you arrive at the registration desk, we will have you sign consents and copy your photo id and insurance card(s).
2. What services/appointments qualify for on-line pre registration?
Scheduled outpatient services at HMH qualify for web based pre registration. These include appointments in Radiology, Laboratory, Nutrition, Surgery, and Maternity.
Emergency visits are not accepted for online pre-registration. If this is an emergency, please dial 911.
3. What if I don't pre-register?
While it is not required that you pre register for outpatient services, doing so may expedite your visit by eliminating a complete review of your demographic and insurance data at the time of your appointment.
Telephone pre-registration also remains an option for those patients who prefer. Pre-registration can be reached at (270) 706-5708 between the hours 11:00 a.m. - 7:30 p.m., Monday through Friday.
4. I don’t have a doctor? Can I still receive services?
Outpatient procedures or exams require a physician order. HMH offers a physician referral service. For more information, see Find a Physician
on the HMH website or call HMH Referral Service at (270)706-1740.
5. Why does my information have to exactly match what is on file at Hardin Memorial Hospital?
HMH uses medical record number to identify patients. If information submitted on a pre registration form does not exactly match what is on file at HMH, a new medical record number may be assigned causing incomplete medical records and inconsistencies.
6. What do I need to bring with me when I come for my appointment?
Please bring the physician order, because without an order, tests cannot be performed. Your physician/physician office may have arranged to send the order directly to HMH. Insurance card(s) and advance directive information should be brought on day of service. You will still need to go to registration area to sign consent forms prior to your procedure.
