Community Health Clinic of Hardin and Larue Counties

The Community Health Clinic of Hardin and Larue Counties is a charitable healthcare organization that provides basic medical, laboratory, pharmaceutical, and dental services, free of charge, to the residents of Hardin and Larue counties who are employed yet incapable of affording health care services.
Clinic Hours of Service:
Tuesday Evenings 5:00 p.m. – 9:00 p.m.
Thursday Evenings 5:00 p.m. – 9:00 p.m.
Services available at the clinic:
Basic Medical Care
Laboratory Services
Pharmacy Services
Diagnostics Referral
Basic Dental Care
Limited Specialist Referral
Durable Medical Equipment
Persons eligible for services include:
Persons gainfully employed who meet the income criteria and their dependents
Persons who are full-time students
Persons receiving disability income who do not have health insurance and their spouses
Persons unemployed for less than 12 months
Persons claiming child support as sole source of income
Persons moving to Kentucky awaiting Medicaid Benefits
Exceptions are made by the Medical Director e.g. unemployed persons with serious chronic health problems
Persons ineligible for treatment include:
Persons with any medical insurance
Persons with military identification
Persons with Medicare, Medicaid, and those on Workers’ Compensation
Persons unemployed longer than 12 months
Persons who exceed the maximum income for household size per income eligibility criteria
Persons who are treated by private physicians and referred to the Community Health Clinic for prescribed diagnostic studies, medications, or treatment
Note:
Persons living in the household who are not related and are not dependents will not be considered
to qualify or disqualify others in the home
New patients must bring:
Identification
Proof of residency
Proof of household income
Services NOT available at the clinic:
The clinic does not provide treatment to persons residing outside Hardin and Larue Counties.
The clinic does not fill prescriptions written outside the clinic
The clinic does not dispense narcotics (pain medication).
The clinic does not perform physical examinations for the purposes of employment, school, athletics, workman’s compensation, or disability claims.
The clinic does not assume financial responsibility for any medical expenses patients may incur as result of recommended diagnostic procedures, treatment, or evaluation (e.g. X-rays, CT scans, biopsies, surgery).
