  Cost: $10 per person (cash or check. Check made out to HMH) Athletes must attend during the time his/her school is assigned! For a physical form, please go to www.khsaa.org/forms/ge04.pdf Physicals are open to incoming 7th grade through High School Senior athletes and band members. The first page of the form must be filled out and a parent/guardian must sign the back page prior to your physical! • HARDIN MEMORIAL HEALTH'S DAY OF SERVICE AT SPRINGHAVEN, INC

  On Friday, April 26th 2013, SpringHaven, Inc. Domestic Violence Shelter welcomed approximately 125 volunteers from Hardin Memorial Health’s (HMH) Leadership Development Institute. The management staff of HMH donated $6000 to fund the day’s projects. The day of service, coordinated by United Way of Central Kentucky, will include volunteer teams tackling building improvements and landscaping, as well as offering direct client services like interview skills and health screenings on the HMH’s Wellness on Wheels (WOW) mobile health unit.
