What you should know:
The medical record is considered the property of Hardin Memorial Hospital, and shall not be taken from the facility except by court order. You have the right to obtain a copy of your medical records. In order to protect your privacy; we must have your written permission on file before release. For security purposes a picture ID is required to release any records.

You may obtain a copy of your medical records from the Health Information Management Services (HIMS) department, also known as Medical Records. To locate HIMS, enter through the front entrance by the Gift Shop, veer right and follow the hallway around the corner and past the Radiology department. Come to the first intersection and turn left, take elevator C or D to the 2nd floor, take a right off the elevator, and we are located at the end of that hallway on the left (MAP PDF).

You may download a copy of the Hardin Memorial Hospital Authorization For The Use And Disclosure Of Health Information form and then fill it out and mail, fax, scan, or hand-deliver your form. Incomplete forms may be returned to you for completion. By law, no information can be released unless the form is properly signed. You may view the General Guidelines for obtaining a copy of medical records. If you cannot download the authorization form, please call (270) 706-1691 and a copy will be sent to you.
