Patient Rights

Patients at Hardin Memorial Health have the right to fair compassionate care at all times and under all circumstances. This is one of many rights provided to HMH patients. Download a full list of patient rights.

MedicalRecords

About Patient Medical Records:
• Medical records are considered the property of Hardin Memorial Hospital
• Records cannot be taken from the facility except by court order.
• Patients have the right to obtain a copy of their medical records.

How to Request a Copy of Medical Records:
• In order to protect patient privacy, written permission must on file before release.
• A picture ID is required to release any records.
• A completed HMH Authorization for the Use and Disclosure of Health Information must be mail, fax, scan, or hand-deliver to Health Information Management Services (HIMS).

Language Services

Hardin Memorial Hospital provides language interpretation services to the limited English proficient/deaf or hard-of-hearing patients and their representative free of charge.

Patient Financial Services

Advance Directives

What is an advance directive? Advance directives (PDF), also known as living will, personal directive, medical directive or advance decision, is a legal document in which a patient specifies what actions should be taken for their health if they are no longer able to make decisions for themselves due to a health-related issue.
While advance directives are not required for care, federal law mandates that hospitals and other healthcare facilities inform patients about advance directives when they are admitted.
