Multi-Disciplinary Breast Program:  Coordination of Care

When diagnosed with breast cancer, the unknown can be terrifying. That’s why at Hardin Memorial Health we have someone to help you navigate the many aspects of your treatment and care.

Nurse Navigator Audrey Cleaver, BSN, RN offers a hand to hold and an ear to listen, as well as extensive knowledge of HMH’s Multi-Disciplinary Breast Program. Our program provides top treatment options close to home, but we recognize that following the ins and outs of each can be overwhelming.

“My job is to decrease anxiety,” Audrey said. “I can help explain procedures, share resources and offer comfort when needed.”

Audrey is available to our breast cancer patients, whether you need a shoulder to lean on, procedure explanations or outside resources such as travel arrangements to treatments.

“I am a familiar face that is there every step of the way,” said Audrey. “My main job is to alleviate worry, provide a personal component to treatment and show our patients that they are in the right place.”

Having a nurse navigator available to patients is one of the many ways HMH seeks to ensure patients receive the best possible care, and to show that we see the person behind the treatment.
